How does it work?

My assessment aim to understand, diagnose correctly, and offer a clear, practical management plan tailored to each individual. What I offer –

First Consultation : Home or Clinic

First consultation is arranged within 24 hours. I meet the patient either at home or in clinic. I make a holistic assessment by taking a detailed history, including memory concern, medical background, daily functioning, social context and input from family or carer.

Detailed Physical Examination

I conduct a structured physical and neurological examination, including cognitive screening, gait and balance assessment, frailty scoring, and system review to inform diagnosis and to make a holistic care plan.

03.

Cognitive test & Other Investigation

Relevant tests are requested to reach a final diagnosis which may include blood tests and imagings. If dementia is suspected then, Full cognitive and other functional tests are performed.

04.

Understanding the Diagnosis and Next Steps

After explaining the diagnosis, I work with you and your family to create a care plan that meets your need. I share the final report with your GP and you also get a copy .

After receiving a request, I will speak to you to discuss ‘appointment’ options and will help to make a choice about the place of assessment.

Most patients prefer home assessment due to health issues. It also helps me in assessing in own environment which is more relaxing and less stressful. This can be arranged within 24 hours.

You can be seen at home wihtin 24 hours. I will visit you as convenient and will make full holistic assessment.

Hospital Consultation

You can choose to be seen at Claremont or Thornbury hospital provided a consultation room is available.
