Pre‑Consultation Questionnaires for Memory Assessment

Please complete these before your appointment

To make your consultation as focused and useful as possible, we ask that the patient and, where possible, a relative or carer complete the relevant questionnaires in advance.

These provide essential information about memory changes, mood, anxiety, and day‑to‑day function, allowing us to use your appointment time for assessment, explanation, and planning.

What you need to do

  • The patient should complete the patient questionnaires below
  • A relative, family member, or carer should complete the informant questionnaires
  • Please complete them at least 24 hours before your appointment, where possible

You will have an option to ‘Stop and Resume’ if you need extra to complete. You will get the link by email to complete the form.

  • If help is needed with the online form, a relative may assist
  • Patient forms should reflect the patient’s own feelings
  • Carer forms should reflect the carer’s observations

Please still attend your appointment. We can go through the assessment forms during the consultation if needed.
