Home visits
Consultant home visits in Sheffield
I’m Dr Pravin Jha, a consultant geriatrician. I visit older people at home within 50 miles of Sheffield, often the same day. A home visit suits people who are frail, housebound, recently out of hospital, or unsettled by unfamiliar places, and it shows me how someone is really managing day to day. A new home visit is £500 for 60 min (up to 10 miles), including a written report to you and your GP.
Why a home visit
Who a home visit is for
Seeing someone at home often gives a clearer picture than a clinic appointment. I can see how they move around, manage stairs, take their medicines and cope with daily tasks, and talk with the family members or carers who know them best.
- people who are frail or housebound
- people with memory problems or dementia who find new places unsettling
- recurrent falls, or worries about safety at home
- recent discharge from hospital
- families who want a senior opinion quickly
Where it would help, a private physiotherapist who works with me can also visit to assess walking and balance, and other needs at home.
A home visit can cover
- Memory and thinking
- Medicines and side effects
- Mobility, falls and safety at home
- Blood pressure, heart and stroke risk
- Parkinson’s disease
- Daily living and support needs
What to expect
What happens at a home visit
Before the visit
- Have your current medicines or prescription list ready.
- Gather recent hospital discharge letters and test results. Many are in the NHS App.
- A GP letter summarising your medical history helps, but is not essential.
The visit
I take a full medical history, with your family or carer if you wish, and carry out a full examination.
Explanation and plan
I explain the likely causes of the problem and plan any tests and treatment with you. Family and carers are often a great help in putting the plan into practice.
Report
I send a written report to your GP, with a copy to you.

Changes and cancellations
To change or cancel a visit, call or text 07855 740098 or email admin@geriuk.com. Occasionally I may have to rearrange a visit because of unforeseen circumstances; if so, I will visit at the earliest opportunity.
A home visit is not an emergency service
If someone is seriously unwell, call 999. For urgent advice, call NHS 111. Do not wait at home for a planned visit.
Questions about home visits
How do I book a home visit?
Send the booking form or call. I then call you to discuss what is needed and confirm the fee, date and time.
How quickly can you visit?
Often the same day.
Which areas do you cover?
Anywhere within 50 miles of Sheffield, including Barnsley, Rotherham, Doncaster, Chesterfield, Dronfield, Worksop, Wakefield and north Derbyshire. I do not hold clinics in these towns: I come to you at home, and clinic appointments are at Spire Claremont Hospital, Circle Thornbury Hospital and the LivingCare Diagnostic Centre in Sheffield. Travel is included up to 10 miles from Spire Claremont; beyond this, £100 per extra 10 miles applies. More detail for families outside Sheffield: memory assessment at home across South Yorkshire and Derbyshire.
How much does a home visit cost?
A new home visit is £500 for 60 minutes and a follow-up is £300 for 30 minutes, plus travel. A memory assessment at home is £900. All include a report to your GP.
Can a relative be there?
Yes, and it often helps. Family and carers can describe changes and help with the plan afterwards.
Do I need a GP referral?
No. You can book directly. A GP letter can be helpful but is not required.
