What should you expect from home visit?
During the visit, I will collect full medical information from you. Then, I will do full medical examination. I will explain you possible causes of your medical issues and will plan investigation and treatment.
Because of the complexity of some situations, the involvement of family or caregivers is very helpful in order to assist you in the implementation of the treatment plan.
What can you do before the appointment?
- Ask your GP to issue you a referral letter summarising your medical history and current issues.
- Arrange copies of discharge summaries from any recent hospitalisation and any special tests that you have undergone. Look at your NHS app.
- Provide your current prescription or gather the medicine list to review .

Terms and conditions
Once you have requested an appointment, you will be contacted to discuss your need and will be advised about total fee.
Fee for a new home visit consultation depends upon total time of consultation and your distance from Claremont Hospital, sheffield S10 5UD.
Consultation fee for general medical issues for a home visit starts at £400.
- £400 for 45min and then EXTRA £100 is payable for every 15 min.
- If you live >10 miles from the hospital, then, EXTRA £100 is payable for every 10miles.
Depending upon complexity, it can take 45-60min to holisticly assess you (£400-500)
Consultation fee for memory assessment for a home visit is : £800
- Session 1: Consultation = 90 min (£700).
- Session 2: Analysis of information, discussion of diagnosis and preparation of report = £100.
- If consultaion exceeds 90 min then additional £100 is payable for every 15 min.
- If you live >10 miles from the hospital, then, additional £100 is payable for every 10miles.
Holistic assessment of memory takes longer due to associated complex medical issues.
I will also send you an invoice which you can pay by online banking. You can also pay by cheque or cash.
You should not wait at home if you have any medical emergency and should call 111 for help.
Due to unforseen circumstances, I may have to cancel your scheduled appointment but will arrange to visit you at earliest.
