What should you expect from home visit?
During the visit, I will collect full medical information from you. Then, I will do full medical examination. I will explain you possible causes of your medical issues and will plan investigation and treatment by liaising with your GP.
Because of the complexity of some situations, the involvement of family or caregivers is very helpful in order to assist you in the implementation of the treatment plan.
What can you do before the appointment?
- Ask your GP to issue you a referral letter summarising your medical history and current issues.
- Arrange for medical records to be made available in advance for the review.
- Arrange copies of discharge summaries from any recent hospitalisation and any special tests that you have undergone.
- Provide your current prescription or gather all the medicines you are taking to review .

Terms and conditions
Once you have requested an appointment, you will be contacted to discuss your need and total fee to pay.
Fee for a new home visit consultation depends upon total time of consultation and your distance from Claremont Hospital, sheffield S10 5UD.
Consultation fee for general old age issues for a home visit is : £400-500
- £400 for 45min, If consultaion exceeds 45min then EXTRA £100 is payable for every 15 min.
- If you live >10 miles from the hospital, then, EXTRA £100 is payable for every 10miles.
Depending upon complexity, it can take 45-60min to holisticly assess you.
Consultation fee for memory assessment for a home visit is : £700
- Session 1: Consultation = 75 min (£600).
- Session 2 (Telephone): Analysis of information and discussion of diagnosis = 15 min (£100).
- If consultaion exceeds 75 min then EXTRA £100 is payable for every 15 min.
- If you live >10 miles from the hospital, then, EXTRA £100 is payable for every 10miles.
Assessment of memory takes longer and more if you have associated complex medical issues.
Your fee includes preparation of a final report and cooordinating care with the GP within 48 hours. You will also receive a copy of the report by email.
I will also send you an invoice which you can pay by online banking. You can also pay by cheque or cash.
You should not wait at home if you have any medical emergency and should call 111 for help.
Due to unforseen circumstances, I may have to cancel your scheduled appointment but will arrange to visit you within 24 hours.
