🧠 Memory & Cognitive Assessments

I offer a comprehensive and holistic assessment for memory concerns, cognitive decline, and geriatric issues including falls, frailty, Parkinson’s disease, continence problems, and TIA/stroke recovery.

No referral is needed. You can book directly. But, a referral from GP helps which provides your medical history, recent tests result and current medications.

You will have a  detailed clinical consultation, cognitive testing, mood assessment and full medical examination.

I will also review your medical history, current medications, and functional status. If needed, I may recommend blood tests, ECG annd brain imaging.

Yes, I can assess for and diagnose different types of dementia including Alzheimer’s disease, vascular dementia, and frontotemporal dementia, and offer ongoing care planning and advice.

A new memory assessment consultation takes longer and depends upon complexity of medical issues. Usual time duration is around 90 min.

Yes, I provide formal mental capacity assessment for decisions such as managing health and welfare, managing finances, independent living, driving, and Power of Attorney evaluations.

I offer a clear management plan and advice on lifestyle and medications.

Yes, I specialise in managing frailty and multimorbidity in older adults, often coordinating with other specialists and carers to provide holistic care.

I manage cognitive issues, falls, frailty, incontinence, polypharmacy, osteoporosis, Parkinson’s disease, stroke, unexplained weight loss, and mobility decline.

Yes, I offer assessment for Parkinson’s diagnosis and help to optimise medication, reduce falls risk, and improve quality of life.

Absolutely. In fact, I encourage family members or carers to attend, especially during memory or cognitive assessments.

I’m based in Sheffield S10, South Yorkshire.

Yes, home visits may be arranged for patients with mobility issues or significant frailty. I cover a wider area and fee depends upon the travelling distance. Please refer to my Consultation fee webpage section.

Yes. Video consultations are available for patients across the UK, where clinically appropriate.

No, I do not work with insurance companies.

I aim to offer appointments within 24 hours.

But, for any emergency, I would suggest to call 999 or visit local Accident & Emergency.

You can arrange a home visit by filling up online form. You can book a clinic appointment by calling Spire Claremont Hospital (0114 263 0330) or Circle Thornbury Hospital (0114 266 1133) directly.

I aim to see you at home within 24 hours. Clinic appointment at Claremont can also be arranged in 1-2 days.

💷 Fees & Payment

Please refer to my ‘Consultation fee’ webpage section.

Assessment in your own home will cost more and depnds upon your distance from Claremont Hospital (see below).

  • New medical consultation (Clinic):  £300 for 45 min and then, £100/15 minutes
  • Memory assessment fee (Clinic): £700 up to 90min
  • Follow up consultation (Clinic): £200 for 20 minutes (up to 3 months)
  • Virtual Consultation: £200 for 30 min
  • Home visit fee: Additional £100 for every 10 miles (in addition to clinic fee)

Your total fee includes preparation of a final report and cooordinating care with the GP within 48 hours. You will also receive a copy of the report by email.

I send an invoice with consultation letter. You can pay it by online banking. You can also pay by cheque or cash.
